Transforming Care

The national Transforming Care programme aims to help people with learning disability and complex needs move out of hospitals and into their own homes, in their own communities with the support they need.

At Avenues we have a long track record of working with commissioners to develop new services for people who need a different type of support to what they currently receive. Our origins lie in supporting people with complex needs following the closure of long-stay hospitals.

What we do:

  • Develop services according to local need
  • Agree models that will deliver sustainable longer-term support, increasing people’s independence and preventing placement breakdown
  • Work with housing partners so that properties reflect individual support plans allowing for the reduction of support over time
  • Consider how environmental design can both reduce behaviours that challenge and enable people to live independently and be active in their local communities
  • Involve people we support and their families so we understand their needs, preferences and goals, and then personalise their support accordingly.

“Every morning when I walk down the stairs into my living room I feel happy and look forward to starting the day. It’s not always easy, but now I have the best support I’ve ever had.” Jackie
