Transforming Care


The national Transforming Care programme aims to help people with learning disability and complex needs move out of psychiatric hospitals and into their own homes, in their own communities with the support they need.

At Avenues we have a track record of working with commissioners and families to develop new services for people who need to make this transition. We began our work as a charity supporting people with complex needs following the closure of long-stay hospitals in the 1990s.

What we do today:

  • Develop services according to local need
  • Agree models that will deliver sustainable longer-term support, increasing people’s independence and preventing placement breakdown
  • Work with housing partners so that properties reflect individual support plans allowing for the reduction of support over time
  • Consider how environmental design can both reduce behaviours that challenge and enable people to live independently and be active in their local communities
  • Involve people we support and their families so we understand their needs, preferences and goals, and then personalise their support accordingly.
